Welcome to the website of the Kingston Bayside Primary Care Partnership (KBPCP).

 

Our partnership offers a strategic link for the planning and provision of health services to member agencies within the cities of Kingston and Bayside in Melbourne's South East.

 

Primary Care Partnerships are an initiative of the State Government to facilitate the development of stronger partnerships, support service coordination and integrated chronic disease management and promote the integration of health promotion across agencies .


Service Coorination Tool Template (SCTT) Revision Project

  Upcoming events

The current cycle of revision of the SCTT, known as the SCTT 2012 revision process, has commenced. In addition to addressing the high priority changes that were unable to be addressed through the SCTT 2009 revision process, a number of major pieces of work are being undertaken.

 

An online electronic feedback form has been added to the SCTT 2012 Revision Project website. Practitioners, agencies, peak bodies, and others are invited to submit changes through the feedback form for consideration as part of the SCTT 2012 Revision Project. The feedback form will be open through to the end of December 2010.

The ABC's of Gambling Counselling
In Australia, problem gambling is more common than stroke and coronary heart disease, yet only about 10% of
people with a gambling problem seek formal help.

 

Gambler's Help Southern is pleased to offer counsellors, social workers, outreach workers, psychologists, case
managers, GPs, nurses and other allied health professionals a training opportunity to increase their
knowledge and skills to work with clients whose lives are adversely affected by problem gambling.
