Kingston Bayside PCP
The Kingston Bayside Primary Care Partnership (KBPCP) is a voluntary alliance which engages over forty government, health and support care providers and community organisations providing services to the cities of Kingston and Bayside.
Our 2009 - 2012 Strategic Plan describes how service providers, communities and governments will work together to plan, coordinate and deliver services to:

Contribute to the health and well being of all people in our community and help alleviate the burden of disease |

Improve people’s experience of and access to primary care services |

Strengthen health promotion and community building |
• |
Reduce preventable hospital admissions by responding to the early signs of disease and to people’s need for support |
To do this, Kingston Bayside Primary Care Partnership is focusing on the five priority strategic areas for 2009 -12:

Victorian Government PCP Strategy
The Victorian Government firmly believes in a stronger role for the PCP Strategy as an effective means for strengthening the primary health care sector. Building a more effective primary health care system for Victoria remains a key government commitment, which it will continue to pursue in partnership with the sector. The Primary Care Partnership (PCP) Strategy encourages member agencies to work together to develop a stronger, more integrated community based health and community service sector.
The Primary Care Partnership Strategy aims to improve the overall health and wellbeing of Victorians by:

Improving the experience and outcomes for people who use primary care services |

Reducing the preventable use of hospital, medical and residential services through a greater emphasis on health promotion programs and by responding to the early signs of disease and/or people's need for support. |
Services have come together to look at the way they work, reducing duplicated practices while increasing understanding of referral pathways and how to improve service coordination and deliver better integrated health promotion (IHP) practice.
