Integrated health promotion: A practice guide for service providers, is a resource kit produced by the Primary and Community Health Branch in partnership with the Public Health Branch (Rural and Regional Health and Aged Care Services Division).This guide is a practical resource to support agencies and organisations to plan, deliver and evaluate effective integrated health promotion programs.

Planning for effective health promotion evaluation, May 2005
This resource will assist agencies, organisations and partnerships to evaluate more effectively their health promotion programs/plans. It supports the principles of integrated health promotion and, in doing so, considers the needs of all parties involved in planning, delivering and evaluating health promotion programs. The resource provides a framework for planning an effective approach to evaluating health promotion practice, and is consistent with the current department integrated
health promotion policy context and current planning and reporting requirements for the branch's funded health promotion programs.

Evaluation framework for health promotion and disease prevention projects
This document forms the basis of the evaluation framework for health promotion and disease
prevention programs. It is complemented by an example plan and a list of agreed indicators.
These have been developed for nutrition, physical activity and obesity projects in the first
instance but it is envisaged that, in future, an agreed list of indicators will be developed for
additional risk factors.

Community Indicators Victoria
Community Indicators Victoria provides a community wellbeing indicator framework with local level data.
Wellbeing indicators are a democratic resource for engaging citizens and communities in informed discussions about shared goals and priorities, a policy resource guiding evidence-based planning and action to address the issues identified as important by communities, and a reporting resource tracking and communicating progress towards agreed goals and outcomes.
CIV is a collaborative project hosted by the McCaughey Centre, School of Population Health, University of Melbourne.

VicHealth Partnerships Analysis Tool for Partners in Health Promotion
Based on the evaluation of a range of initiatives undertaken to promote mental health and wellbeing, John McLeod, on behalf of VicHealth, produced The Partnerships Analysis Tool to facilitate partnerships across sectors. It is a resource which (a) assists organisations to develop a clearer understanding of the range of purposes of collaborations, (b) reflects on the partnerships they have
established and, (c) focuses on ways to strengthen new and existing partnerships by engaging in discussion about issues and ways forward. |
