Our Work - Service System Development - e-Referral

Electronic referral (e-referral) is being undertaken in agencies and some hospital based services throughout Victoria. In some areas, such as the Western Metro Region and the Eastern Metro Region, e-referral is well established, with pockets of “best practice” in place. The e-referral systems developed as part of the PCP strategy are based on the statewide Service Coordination Tool Templates.

Kingston Bayside PCP endorses the use of Connectingcare to facilitate best practice in e-Referral. Over one hundred primary care, community health and acute agencies use the Connectingcare services directory to find the most up-to-date information on over 6000 services across Victoria. The e-referral system enables client information to be sent and received using encrypted email. Connectingcare uses existing Internet-based technology to create a secure platform for referral without the cost of additional software or equipment.
