Our Work - Service System Development - Overview
Service System Development encompasses the areas of Service Coordination and Integrated Chronic Disease Management.
Service coordination reform aims to place consumers at the centre of service delivery - ensuring that they have access to the services they need, opportunities for early intervention and health promotion and improved health outcomes. Service coordination is facilitated by Primary Care Partnerships where agencies come together to agree on how they will coordinate their services so that consumers experience a health system that works together.
Integrated Chronic Disease Management includes the following:

planned and proactive care intended on keeping people as well as possible rather than responding to an illness |

empowering, systematic and coordinated care that includes regular screening, support for self management, assistance to make lifestyle and behaviour changes |
| • |
care that is provided by a range of health services and practitioners (eg. GPs, podiatrist, physiotherapist, counsellor, dietitian, nurse, specialist, dentist) |
| • |
care that is provided over time through the stages of disease progression |
A priority for PCPs is their support to enhance the provision and coordination of self-management approaches.
